i do the same as FrankG plus i use the insert as a pattern and cut a piece of old wool army blanket and glue it to the insert with contact cement, helps keep your feet warm in the winter.

All my footwear (even house slippers) have to have something to support my arches.  The result of LOTS of running when I was younger.  I find that my prescription orthotics, or some store boughten insoles work very well at cushioning my feet from all the little gremlins on the ground.  I wear only center seam mocs, and they work fine.  Megwetch, Sinnanatha

Try the Rhino-tuff insoles by mitchellance.com.  These have a steel piece covered with an insole and are designed to resist punctures.  They work real well in my knee-high Minnetonkins.

Hey Ned.  I have used dr scholls and another sole with arch support in it at the same time.  Never had any trouble with my feet hurting...course noone told me this trick the first year out and I had major sore feet. :)
